THE CITY OF w O <br /> CITY COUNCIL AGENDA REPORT I <br /> pL E ASANTON. <br /> June 16, 2015 <br /> Community Services <br /> TITLE: APPROVE THE SERVICE AGREEMENT WITH THE SENIOR SUPPORT <br /> PROGRAM OF THE TRI-VALLEY IN THE AMOUNT OF $88,000 FOR <br /> SENIOR SUPPORT SERVICES <br /> SUMMARY <br /> Since 2000, the City has provided a range of health and social services directed toward <br /> senior citizens who reside in Pleasanton. This has been coordinated through an <br /> agreement with Senior Support Programs of the Tri-Valley. The current agreement will <br /> expire on June 30, 2015, and in anticipation of that expiration the City conducted a <br /> Request for Proposal process to select the most qualified vendor and expand this <br /> program. <br /> On March 23, 2015 a sub-committee comprised of two (2) Human Services <br /> Commissioners and City staff was formed to review proposals and interview the <br /> applicants. Based on that process, the sub-committee is recommending awarding an <br /> agreement to the Senior Support Program of the Tri-Valley for FY 2015/16. Services to <br /> be provided include: Friendly Visiting, Case Management, In-home Caregiver Registry, <br /> Family Caregiver Support, Alcohol and Drug Abuse Prevention, Education and <br /> Outreach, Exercise and Nutrition Classes, Health Screenings, Information and Referral <br /> Services. This agreement also includes providing one (1) additional day of programs <br /> and services, as well as additional Foot Care Services. <br /> RECOMMENDATION <br /> Approve the Service Agreement with the Senior Support Program of the Tri-Valley to <br /> provide Senior Support Services. (Attachment 1) <br /> FINANCIAL STATEMENT <br /> The proposed Services Agreement with the Senior Support Program of the Tri-Valley is <br /> in the amount not to exceed $88,000 annually. Funding is included in account #011501 <br /> as part of the proposed FY 2015/16 Operating Budget. <br />
