Section 12. Health and Welfare Coverage <br /> <br />12.1 <br /> <br />Hospital and Medical <br /> <br />Effective January 1, 1982, the City shall contribute an amount <br />necessary to provide hospital and medical care benefits under the <br />present Blue Cross Plan or Kaiser S-1 Plan for the individual <br />employee and a portion of the amount necessary to provide cover- <br />age for eligible dependents; provided, however, that in no event <br />shall the City be obligated to contribute an amount in excess of <br />One Hundred Nineteen Dollars ($119.00) per month per employee to <br />provide the aforesaid coverage. Effective July 1, 1982, the City's <br />maximum monthly contribution for said medical coverage shall be <br />increased to One Hundred Thirty Five Dollars ($135.00). Effective <br />July 1, 1983, the City's maximum monthly contribution for said <br />medical coverage shall be increased to One Hundred Forty Eight <br />Dollars ($148.00). <br /> <br />For the purpose of providing the aforementioned medical coverage <br />to employees who retire for service on or after their fiftieth <br />(50th) birthday, the City shall contribute an amount equal to one- <br />half (1/2) the monthly premium for said health coverage; in no <br />event, however, shall said City contribution exceed Fifth Dollars <br />($50.00) per month per employee. Said monthly City contribution <br />shall terminate on the employee's sixty-second (62nd) birthday. <br /> <br />12.2 Dental <br /> <br />Effective January 1, 1982, the City shall contribute an amount <br />· necessary to provide dental care benefits under the present <br />Bankers Life Nebraska Plan for the individual employee and eligible <br />dependents; provided, however, that in no event shall the City be <br />obligated to contribute an amount in excess of Thirty Four Dollars <br />and Fourteen Cents ($34.14) per month per employee to provide the <br />aforesaid coverage. Effective January 1, 1983, the City's maximum <br />monthly contribution for said coverage shall be increased to Thirty <br />Seven Dollars ($37.00) per month per employee. <br /> <br />12.3 <br /> <br />Life, Accidental Death & Dismemberment end Long Term Disability <br /> <br />The Cit~ shall contribute an amount necessary to provide life, acci- <br />dental death and dismemberment, and long term disability insurance <br />under the Ctty's present Standard Insurance Company Group policy for <br />the individual employee; provided, however, that in no event shall <br />the City be obligated to contribute an amount in excess of One Dollar <br />and Seven Cents ($1.07) per One Hundred Dollars ($100) of monthly <br />salary of the first $2250. of monthly salary not to exceed a total <br />contribution of Twenty-Four Dollars and Eight Cents ($24.08) per <br />month per employee. <br /> <br />Section 13. Retirement <br /> <br />Effective January 1, 1982, the City shall pay the employee's contri- <br />bution to the Public Employees' Retirement System (PERS). The amount <br />of said City payment shall equal but not exceed nine percent (9%) of <br />the employee's compensation subJect to retirement contributions. <br /> <br />-12- <br /> <br /> <br />
