7/18/83 - 26630 <br /> <br />The total number of Centrex CO primary, semirestricted and <br /> <br />interior lines, hereinafter referred to as Centrex CO <br />lines, installed on September~ 1983, is the total number <br />of such lines for which billing will be rendered on the <br />date of the first billing in September, 1983. <br /> <br />Customer and Utility agree that Exhibit A shall reflect the <br />actual billing of the service items contained in Exhibit A <br />for Centrex CO billed and such September, 1983 billing or <br />any adjustment thereof mutually agreed to by Customer and <br />Utility. Exhibits A and B exclude any future access line <br />charges which may be authorized by the Federal <br />Communications- Commission and/or the Public Utilities <br />Commission of the State of California. <br /> <br />All facilities other than those stated in Exhibit A are <br />furnished in accordance with prevailing tariff rates, <br />charges and conditions. Customer may increase or decrease, <br />subject to availability, the quantity of such facilities <br />initially ordered and/or may request, if applicable, such <br />facilities to be moved, altered or revised by informing <br />Utility of the desired change. Utility's Service Order <br />reflecting such changes and the in-service dates therefor <br />will be made part of this Agreement and will be kept on <br />file by Utility at Utility's office located at 582 Folsom <br />Street, First Floor, San Francisco, California. The <br />charges for such changes will be at the rates applicable at <br />the time the change is performed. <br /> <br />A minimum ninety percent (90%) sustained level of Centrex <br />CO lines must be maintained during the term of this <br />Agreement. If lines are disconnected below this minimum, <br />Customer may elect to continue paying for the lines at the <br />ninety percent (90%) minimum at rates set forth in <br /> <br />-2- <br /> <br /> <br />
