WHEREAS, <br /> <br /> WHEREAS, <br />a program, and <br /> <br />This agreement is made and entered into this date, . ......... by and <br />between Valley Memorial Hospital, hereinafter referred To as "Hospital", Regional <br />Medical Systems, hereinafter referred to as "RMS", and the fire departznents of the <br />City of Livermore, the City of Pleasanton, the City of Dub]in, and [awrence Li!? <br />Pore Lab, herein&fter referred to as "Valley Fire Depart;nents or VFD", and <br /> <br /> WHEREAS, The VFD are desirous of a California Emergency Medical Technician <br />!A (hereinafter called EMT-1A) recertification program, and <br /> <br /> RMS is desirous of a EMT-1A recertification program, and <br /> <br /> it is beneficial to the Hospital and its cormunity to have such <br /> <br /> WHEREAS, the Hospit~ is designated in the Ala~T,eda County Emergency Medical <br /> Services System as Base Hospital for Zone IX, and is willing to provide said <br /> recertification program, the parties agree as follows: <br /> Hospital Responsibilities <br /> <br />1, The Hospital will design an EMT-1A recertification program, to be accredited <br /> by the "EMT-1A Approv~ng Authority." <br /> <br />2. The Hospital will provide the primary instructor and administrative coordinator. <br /> <br />3. The Hospital will provide clinical experience as required. <br /> <br />4. The Hospital will provide six (6) EMT-1A recertifica tion programs each year. <br /> <br />5. The Hospital will provide any new training aids and/or equipment needed, that <br /> is not already available from the parties (see Shared Responsibilities). <br /> <br />6. Teaching assistants provided by RMS and VFD shall be approved by the primary <br /> instructor. <br /> <br />RMS Responsibilities <br /> <br />1, RMS will provide an ambulance and a teaching assistant for that part of the <br /> program involving ambulance operations and transportation. <br /> <br /> <br />
