Examples: completion of 19 years = 57% of the single plan rate for the lowest cost HMO and <br />completion of 20 years =80% of the two-party rate for the lowest cost HMO. <br />3. City Contributions <br />Effective July 1st of each year of this agreement, the monthly contribution paid by the City <br />shall be increased by the same increase in medical insurance insurance for the lowest cost <br />HMO single or two-party rate as is provided to active employees as set forth in Section 14.1 of <br />this agreement and as applicable under Section 14.2 G.1. and 2. above. <br />14.3 Dental <br />The City shall contribute an amount necessary to provide dental care benefits for the individual <br />employee and eligible dependents. Effective July 1, 2008 the annual maximum will be <br />increased to $2000 per person per calendar year. Maximum orthodontia coverage shall be <br />$2,000 lifetime allowance. <br />14.4 Vision <br />During the term of this Memorandum of Understanding, the City shall contribute the premium <br />necessary for the VSP Vision Plan family coverage for health insurance participants who are <br />not covered by Kaiser. <br />14.5 LonE Term Disability <br />The City's maximum monthly long term disability insurance premium contribution shall be <br />increased to twenty three dollars ($23.00). The City shall not be obligated to contribute an <br />amount in excess of twenty three dollars ($23.00) throughout the term of this agreement. <br />14.6 Life. Accidental Death and Dismemberment Insurance <br />The City shall contribute an amount necessary to provide $50,000 life and $50,000 accidental <br />death and dismemberment insurance coverage for Association members. The monthly <br />premium for such insurance coverage shall be paid by the City during the term of this <br />agreement. <br />Section 15. Retirement <br />15.1 Payment of Employee Contribution to PERS <br />The City shall pay the employee's contribution to the Public Employees' Retirement System <br />(PERS). The amount of said City payment shall equal but not exceed nine percent (9%) of the <br />employee's compensation subject to retirement contributions. The City shall report as special <br />compensation the dollar value of the City's payment of the employee's nine percent retirement <br />contribution. <br />15.2 PERS Plan Options <br />The City's contract with PERS provides for the following options: Credit for Unused Sick <br />Leave; 3% @50 Retirement Formula; Single Highest Year Option; and the PERS Military <br />Buyback Provision. <br />19 <br />
