The immediate family shall consist of the spouse, domestic partner, <br />children, children of a domestic partner, parents, or other individuals whose relationship to <br />the employee is that of a dependent or near dependent. <br />This provision does not apply if the family sickness or disability occurs while the <br />employee is on vacation leave. <br />In order to receive compensation while absent on sick leave the employee shall cause <br />notification to his/her immediate supervisor or the Chief of Police prior to the time set for <br />beginning his/her daily duties or as maybe specified in Department Policy. <br />When an absence is more than two consecutive full or partial shifts, the employee maybe <br />required to file a physician's certificate or persona] affidavit stating the cause of the absence; <br />except, however, if the employee uses excessive sick leave and/or demonstrates a pattern of <br />sick leave misuse, abuse, or falsification (i.e., using sick leave in conjunction with days off, <br />vacation, holidays, or otherwise) the employee maybe required to file the physician's statement <br />or personal affidavit following an absence of a single work shift or otherwise comply with the <br />City's Attendance Management Program. Any proposed changes to the City's current <br />Attendance Management Program shall be negotiated with the PPOA. <br />12.3 Catastrophic Leave <br />The Catastrophic Leave Policy attached as Exhibit D shall be extended to Association <br />members. <br />Section 13. Leave of Absence <br />13.1 General Leave of Absence <br />Any regular or probationary employee may submit a written request for a general leave of <br />absence up to a maximum of three (3) calendar months. Such written request shall be <br />submitted to the Chief of Police and the Human Resources Manager and is subject to the sole <br />and final approval of the City Manager. The written request must state the reasons for the leave. <br />The Chief of Police may grant a regular or probationary employee a leave of absence without <br />pay, for a period up to two (2) calendar weeks. Said leaves shall be reported to the Human <br />Resources Manager. <br />During said leave, an employee shall not accrue vacation or sick leave benefits or be eligible <br />for holidays, nor shall such time be considered as service time. <br />13.2 Extended Medical Leave <br />Any regular employee who is temporarily disabled from work due to a medical condition may, <br />upon written request to the Department Head and the Personnel Director, be granted by the City <br />Manager, a leave of absence without pay for a period not to exceed three (3) months beyond the <br />exhaustion of any accrued sick and vacation leave, except as maybe provided bylaw. <br />Probationary employees maybe eligible for leave as provided by law. All such leave, paid and <br />unpaid, shall run concurrently with Family Medial Leave, California Family Rights Leave and <br />Pregnancy Disability Leave in accordance with City Policy. <br />14 <br />
