14.2 Dental <br /> <br /> Effective March 1, 1985, the City shall contribute an amount <br /> not to exceed thirty eight dollars and eighty two cents <br /> ($38.82) per month per employee to provide dental care <br /> benefits under the City's present Banker's Life Nebraska <br /> Plan for the individual employer and eligible dependents. <br /> <br />14.3 Life, Accidental Death & Dismemberment and Long-Term <br /> Disability <br /> <br /> The City shall contribute an amount necessary to provide <br /> $20,000 life and $20,000 accidental death and dismemberment <br /> insurance coverage for the individual full time employee. <br /> <br /> Effective March 1, 1985, the City shall contribute an amount <br /> necessary to provide long-term disability insurance under <br /> the present Standard Insurance Company group policy or an <br /> acceptable alternative plan for the individual full time <br /> employee; provided, however, that in no event shall the City <br /> be obligated to contribute an amount in excess of one and <br /> fifty one-hundredth dollars ($1.50) per one hundred dollars <br /> ($100) of monthly salary of the first $2,250 of monthly <br /> salary not to exceed a total contribution of thirty-three <br /> and seventy-five one hundredth dollars ($33.75) per month <br /> per employee. <br /> <br />14.4 Health and Welfare Benefit Continuation <br /> <br /> Employees off work due to verifiable personal illness and <br /> having exhausted their accrued sick leave benefits set forth <br /> in Section 12 shall continue their eligibility for health <br /> and welfare payments in accordance with Section 14. <br /> Employees off work on leaves of absence without pay shall <br /> not be eligible for said health and welfare benefit <br /> payments. <br /> <br />Section 15. Retirement <br /> <br /> The City shall amend it PERS Local Miscellaneous Member <br /> Retirement Plan for the purpose of adopting the PERS "credit <br /> for unused sick leave option." <br /> <br /> Effective March 1, 1982, the City shall pay the employee's <br /> contribution to the Public Employees' Retirement System <br /> (PERS). The amount of said City payment shall equal, but <br /> not exceed seven percent (7%) of the employee's compensation <br /> subject to retirement contributions. <br /> <br /> -21- <br /> <br /> <br />
