In order to receive compensation while absent on sick leave <br /> the employee shall notify his/her immediate superior or the <br /> Personnel Officer prior to or within four hours after the <br /> time set for beginning his/her daily duties or as may be <br /> specified by the Department Head. When absence is for more <br /> than eight (8) hours, the employee may be required to file a <br /> physician's certificate or a personal affidavit with the <br /> Personnel Officer stating the cause of the absence. <br /> <br />Section 13. Leaves of Absence <br /> <br />13.1 Leaves of Absence Without Pay <br /> <br /> The Appointing Authority may grant a permanent or <br /> probationary employee leave of absence without pay or <br /> seniority for not to exceed three (3) months. No such leave <br /> shall be granted except in writing and upon written request <br /> of the employee setting forth the reason therefor. Upon <br /> expiration of a regularly approved leave, the employee shall <br /> be reinstated in the position held at the time leave was <br /> granted. Failure on the part of an employee on leave to <br /> report promptly at its expiration or within a reasonable <br /> time after notice to return to duty shall be cause for <br /> discipline. Department Heads may grant a permanent or <br /> probationary employee leave of absence without pay for not <br /> to exceed one (1) calendar week. Such leaves shall be <br /> reported to the Personnel Officer. <br /> <br />13.2 Jury Leave <br /> <br /> Every employee of the City who is called or required to <br /> serve as a juror shall be entitled to leave from his/her <br /> duties with the City during the period of such service or <br /> while necessarily being present in court as a result of such <br /> call. The employee shall be paid the difference between <br /> his/her full salary and any payment received by him/her, <br /> except travel pay, for such duty. <br /> <br />13.3 Leaves Resulting from Subpoena <br /> <br /> Leave of absence with pay shall be granted to a person while <br /> going to and from court and answering a subpoena as a <br /> witness. The employee shall be paid the difference between <br /> his/her full salary and any payment received by him/her, <br /> except travel pay, for such duty. <br /> <br />13.4 Military Leave <br /> <br /> Military leave shall be granted in accordance with the <br /> provisions of law. All employees entitled to military leave <br /> shall give the Appointing Authority an opportunity within <br /> the limits of military regulations to determine when such <br /> leave shall be taken. <br /> <br /> -19- <br /> <br /> <br />
