ATTACHMENT 5 CONTRACT BERNAL AVENUE BRIDGE AND STREET WIDENING FROM VINEYARD AVENUE TO DEL VALLE PARKWAY PHASE III PROJECT NOS. 995067 AND 005029 AND DEL VALLE PARKWAY SEWER IMPROVEMENTS, PROJECT N0.902028 THIS CONTRACT is made and entered into this day of , 20_ by and between ("Contractor"), whose address is ,and telephone number is PLEASANTON, a municipal corporation ("City"). WITNESSETH: and the CITY OF WHEREAS, the City has awarded to the Contractor a contract for BERNAL AVENUE BRIDGE AND STREET WIDENING FROM VINEYARD AVENUE TO DEL VALLE PARKWAY PHASE III, PROJECT NOS. 995067 AND 005029 AND DEL VALLE PARKWAY SEWER IMPROVEMENTS, PROJECT N0.902028. NOW, THEREFORE, in consideration of the mutual promises set forth herein, the parties agree as follows: Work to be Performed. The work will generally consist of: • Constructing Bernal Avenue Bridge, a four span P/C P/S concrete slab bridge with cast-in-place deck, 284 feet in length and 42 feet wide. • Planting and irrigation along the corner of Bernal Avenue and Vineyard Avenue. • Traffic signal modification for the ultimate roadway alignment at the intersection of Bernal Avenue at Vineyard Avenue. • The work also consists of the widening of Bernal Avenue between Vineyard Avenue and Del Valle Parkway to accommodate the new two-lane bridge and overlay of the existing pavement section between Nevada Court and Tawny Drive. • Constructing 340 feet of 18" PVC sanitary sewers, and 360 feet of 18" HDPE sewer line on Bernal Avenue. • Provide grading, planting, irrigation, and 1 year maintenance services as part wetland and ripaxian mitigation requirements for the subject project.
