At the time of the inspection, items #1,2,3,4,5, and 9, of the January <br />27, 20041etter, were resolved and the UNSAFE notice removed. However, your application for a <br />permit to upgrade your occupancy will need to be resolved in a timely manner to prevent further action. <br />And as stated in our letter of January 27, 2004, the following items are required to be resolved within 30 <br />days of occupancy release, or no later than March 22, 2004. <br />It is unlawful to maintain or use a dangerous building or structure, or dangerous electrical, plumbing or <br />mechanical installation. Specifically these violations include the following: <br />1. Possible overrating and/or overloading of electrical outlets (computers) at back computer <br />room. Eliminate the abundance offlexible cords (extension cords). Flexible or Extension <br />cords shall not be used as a substitute for fzxed wiring. (Permit is required for new wiring. <br />2001 California Electrical Code, sections 210-21, 210-50, 305-6A and 400-8. <br />2. Exposed (unknown type) wires, at locker and large room adjacent to the mat room. Clear <br />area for access and schedule an inspection with the building division to determine required <br />action. (Permit may be required) 2001 California Electrical code, section 300-4. <br />3. Non-functioning GFCI outlets and reverse polarity (improper wiring within box and/or <br />system) at several outlets. Replace and/or repair. This work may be covered under the <br />tenant improvement application. 2001 California electrical code, section 110-3(b). <br />4. Shower installed without proper permits and or inspections. (Permit required). Dublin <br />Municipal Code, section 7.28.280 and 7.40.060. <br />These items may or may not be a part of the improvements to be made under your permit application of <br />November 14, 2004. The above items are not a complete list of the requirements listed in the <br />conditional use permit. Completion of these items does not relieve you of your obligations set forth <br />under the Dublin Municipal Code and the California Building Codes. <br />Page 1 of 2 <br />
