the degree or certificate and the major field of <br /> study. <br /> <br /> B. Evidence of completion of the required accredited semester <br /> units or quarter hour equivalents may consist of any of <br /> the following: <br /> <br /> 1. College transcript of grades. <br /> 2. Official grade slips from each of the courses. <br /> 3. Letter from the college registrar certifying the <br /> completion of each course with a grade of "C" or <br /> better. <br /> <br />19.7 Termination of Educational Incentive Pay <br /> <br /> Educational Incentive Pay shall be terminated at such time as <br /> the employee receiving educational incentive pay is promoted to <br /> or otherwise placed in a position requiring the educational <br /> level which he currently possesses or a higher educational <br /> level. <br /> <br />19.8 Tuition Reimbursement <br /> <br /> A. Only employees enrolled in the Educational Incentive Plan <br /> shall be eligible for tuition reimbursement. <br /> <br /> B. Tuition shall be reimbursed in the amount and at the rate <br /> for internal degrees levied by Hayward State University at <br /> the time of the employee's enrollment in the courses for <br /> which tuition reimbursement is requested. However, the <br /> maximum amount of tuition reimbursement shall not exceed <br /> $300.00 per year per' employee. <br /> <br /> C. Tuition reimbursement shall not be received for any <br /> individual courses (or class) not successfully completed <br /> with a grade of "C" or better. <br /> <br /> D. Employees submitting requests for tuition reimbursement <br /> shall provide the Personnel Officer an official receipt <br /> from the college or university verifying the payment of <br /> tuition and an official transcript of the grades received <br /> for which tuition is requested. <br /> <br />19.9 Book Reimbursement <br /> <br /> Employees enrolled in the Educational Incentive Plan shall be <br /> eligible for the reimbursement of the books required for those <br /> classes taken as part of their approval educational program. <br /> The amount reimbursed shall be the purchase price of the books <br /> less the monies received from their resale. No reimbursement <br /> for books shall be received for any class not successfully <br /> completed with a grade of "C" or better. <br /> <br /> Employees submitting requests for reimbursements for books <br /> shall submit to the Personnel Officer the original purchase <br /> receipt in addition to the receipt showing the monies received <br /> through resale of said books. <br /> <br /> - 26 - <br /> <br /> <br />
