Upon qualifying for and receiving parts A and B of Medicare, the <br />City shall not be obligated to contribute a monthly amount in <br />excess of the monthly premium for the Kaiser S-1 supplement to <br />medicare health plan for employee and one dependent. In the case <br />where the eligible employee is deceased, the amount the City is <br />obligated to pay shall be reduced by 1/2 (one half). If the <br />spouse remarries, the obligation by the City shall terminate. <br /> <br />The City reserves the right to meet and confer with the <br />Union for the purposes of coordinating retired employees <br />hospital and medical care in the event Federal or State <br />legislation is implemented that effects retiree's medical <br />insurance. <br /> <br />The City will contact the health care providers and seek <br />the opportunity to place retirees under the active plans. <br /> <br />15.3 Dental <br /> <br /> Effective July 1, 1988, the city shall contribute an <br /> amount not to exceed forty-two dollars ($42.00) per <br /> month per employee to provide dental care benefits under <br /> the city's present Banker's Life Nebraska plan for the <br /> individual employee and eligible dependents. Effective <br /> January 1, 1989 said maximum contribution shall increase <br /> to forty-five dollars ($45.00) per month. <br /> <br />15.4 Life Insurance <br /> <br /> Effective July 1, 1986, the City shall contribute an <br /> amount necessary to provide life insurance coverage under <br /> the present Standard Insurance group policy for the <br /> individual employee. <br /> <br /> 15.5 Long-Term Disability <br /> <br /> Effective July 1, 1988 the City's provision of long term <br /> disability insurance coverage to employees under the <br /> Canada Life Plan shall terminate and the City shall have <br /> no further responsibility to provide long term disability <br /> benefits except as provided below. <br /> <br /> Effective July 1, 1988 the City shall pay to the Union <br /> the sum of thirty three dollars ($33.00) as the monthly <br /> premium of each employee enrolled in the union's long <br /> term disability plan. Said payments shall be made in <br /> arrears monthly. Should the monthly premium of the <br /> Union's long term disability plan drop below thirty three <br /> dollars per month, the dollar difference shall be paid to <br /> each employee enrolled in the plan in his/her uniform <br /> allowance. <br /> <br /> - 20 - <br /> <br /> <br />
