Section 15. Health and Welfare Coveraqe <br /> <br />15.1 Hospital and Medical Care - Active Employees <br /> <br /> Effective July 1, 1988, the City shall contribute an <br /> amount necessary to provide hospital and medical care <br /> benefits under either the present Blue Cross Plan, <br /> Kaiser S-1 Plan or, Foundation Health Plan I for the <br /> individual employee and a portion of the amount necessary <br /> to provide coverage for eligible dependents provided, <br /> however, in no event shall the City be obligated to <br /> contribute a monthly amount in excess of the monthly <br /> premium for the Kaiser S-1 Health Plan full family <br /> coverage. <br /> <br />15.2 Health & Medicare Coveraqe Retirees <br /> Effective July 1, 1986 employees having retired for <br /> service on or after their fiftieth (50th) birthday <br /> the City shall reimburse upon receipt of proper <br /> documentation an amount not to exceed seventy-five <br /> dollars ($75.00) per month toward the cost of self <br /> procured medical plan coverage. Said reimbursement <br /> shalll cease on the employee's seventieth (70th) <br /> birthday. <br /> <br /> Effective August 31, 1990, for all employees who retire <br /> for service after August 31, 1990 the City shall pay for <br /> each year of service, four percent (4%) of the monthly <br /> premium for employee and one dependent of the City's <br /> current Kaiser S-1 Health Plan coverage. Effective <br /> August 31, 1990 for all employees who retire for <br /> disability after August 31, 1990 the City shall pay a <br /> percentage of the monthly premium for employee and one <br /> dependent of the City's current Kaiser S-1 Health Plan <br /> coverage in accordance with the schedule set forth <br /> below: <br /> <br /> Years of Service City's Contribution <br /> 10 20% <br /> 11 25% <br /> 12 30% <br /> 13 35% <br /> 14 40% <br /> 15 45% <br /> 16 48% <br /> 17 51% <br /> 18 54% <br /> 19 57% <br /> 20 80% <br /> 21 84% <br /> 22 88% <br /> 23 92% <br /> 24 96% <br /> 25 + years of service 100% <br /> - 19 - <br /> <br /> <br />
