Resolution No. 2866 <br />Planning Commission <br />November 12, 1986 <br />2. That there shall be the following uses on the site: <br />1) up to 12,000 sq. ft. in medical office uses <br />(Bldg. 4 and 5), 2) a 4,608 sq. ft. diagnostic lab, <br />3) a 5,184 sq. ft. out-patient surgical unit and 4) <br />a 6,336 sq. ft. exercise/rehabilitation center. <br />The remaining uses shall be non-medical office uses <br />and shall have one parking space per 300 sq. ft. of <br />gross area. <br />3. That the total number of employees and patients for <br />each building shall not exceed the following totals <br />at any given time: 13 occupants for the diagnostic <br />lab, 17 for the out-patient surgical unit, and 21 <br />for the exercise/rehabilitation unit. <br />4. That the Planning Director shall restrict the use <br />or, if necessary, revoke the business license if <br />any of the above conditions are not met. <br />5. That no users of the facility (staff) shall park <br />off-site. <br />6. That except as modified by Conditions 1 to 4 above, <br />all conditions of PUD-82-18-6D to PUD-82-18-6D-2M <br />shall remain in full force and effect. <br />Section 2.
