Exhibit 5-A AMeNDmENT NO. 5 'HACIENDA DRIVE/I-580 INTERCHANGE SCOPE OF SERVICES BACKGROUND During the course of project development~ the CONSULTANT has provided the services as contained in Amendment No. 2 to the Contract for Consulting Services dated October 18~ 1988~ as well as additional services which were outside the scope of that Amendment. The purpose of this AMENDMENT is to recog- nize~ accept~ and compensate the CONSULTANT for the addi- tional work performed that was outside the original project workscope and to authorize the CONSULTANT to proceed with additional work to complete the design of the Hacienda Drive Interchange as described in this AMENDMENT. It ts also recognized that the CONSULTANT has executed the work under an accelerated completion schedule and will continue to operated in this mode to complete the remainder of the assigned work. SCOPE OF SERVICES The CONSULTANTS work authorized by this AMENDMENT includes: TASK 1--WESTBOUND DIAGONAL ON-RAMP (HR-3 RAMP) The CONSULTANT shall add the westbound diagonal on-ramp to the final design plans. Work shall include preparations of alignment, grading, paving, drainage, lighting, signals, signing, and striping plans. Work shall be incorporated into the final interchange design. TASK 2--FRONTAGE ROAD The CONSULTANT shall add the frontage road modifications to relocate the connection of the existing frontage road in the NE quadrant of the interchange. The design of the frontage road will include the horizontal and vertical alignment, roadway drainage, signing, and striping. Completed design plans shall be revised to include the relocated frontage road. TASK 3--UTILITIES The CONSULTANT shall continue providing services to coor- dinate relocation of utilities needed to construct both the Hacienda Drive and Santa Rita Road interchanges.
