To assist us in our efforts, we will need the City to do the following: <br /> <br /> * Identify all welfare benefit plans provided by the City. <br /> <br /> * Identify City practices that provide benefits to employees, such as physical <br /> exams or employee discounts, that are not curren~y in writing. <br /> <br /> * Gather documentation related to the plans, including: summary plan <br /> descriptions, insurance contracts, HMO agreements, personnel policy bullitins <br /> and memos to employees. <br /> <br /> * Identify City procedures for distribution of plan information to participants <br /> upon initial eligibility and after a change in benefits provided under the <br /> plans. <br /> <br /> * Identify effective dates for all benefit plans. <br /> <br /> * Identify non-employee participation in the plans (e.g., Councilmen, <br /> commissioners). <br /> <br /> Methodology <br /> <br />We anticipate spending two hours with you and your staff in an initial meeting to discuss <br />the plans and benefits provided by the City and to review the supporting documentation, <br />the City's practices and its procedures pertinent to those plans. We will conclude the <br />meeting with an agreement on what further information the City must gather. Subsequent <br />meetings will be scheduled as needed. We will prepare a summary similar to exhibit B, a <br />draft wrap-around document, and an opinion letter for your records. I will be assisted by <br />office paralegal staff as needed. <br /> <br /> Fees <br /> <br />Our professional fees are based on our hourly rates and measured strictly by the actual <br />time spent working on behalf of the City. Travel expenses will be billed separately at <br />cost. With the exception of any unforseen legislative modification, we will limit our <br />professional fees to be not more than $4,200 for the services described in this letter. <br /> <br /> <br />
