Employee's Year of Employment Maximum Weeks of Benefit <br /> Second Year Six Weeks <br /> Third Year Four Weeks <br /> Fourth Year Two Weeks <br /> The City, at its discretion, may choose to convert the above income continuation benefit from a <br /> self - insured benefit to an insured benefit. <br /> 6.5(2) Long -Term Disability Insurance <br /> The City shall provide, at its expense, management and confidential employees long -term <br /> disability insurance coverage. The maximum monthly benefit will be $4,000.00 with a 60-day <br /> waiting period. During this plan year, the City will investigate increasing the maximum <br /> monthly long term disability benefit and/or converting the benefit premium from employer paid <br /> to employee paid. <br /> 6.6 Vision Care <br /> Effective September 1., 1997 management and confidential employees and their eligible <br /> dependents enrolled in of the City's Health Net health plan shall be enrolled in the City's Vision <br /> Service Plan. The premium for the Vision Service Plan for full -time employees and their <br /> eligible dependents shall be paid by the City. The prernium paid by the City for regular part - <br /> time management and confidential employees shall be prorated. <br /> Section 7. Leaves <br /> 7.1(1) Observed Holidays <br /> Annual observed holidays are as follows: <br /> A. January 1st <br /> B. The third Monday in January, known as "Martin Luther King Day" <br /> C. The third Monday in February, known as "Washington Day" <br /> D. The last Monday in May, known as "Memorial Day" <br /> E. July 4th <br /> F.
