73 92% <br /> 24 96% <br /> 25 + years of service 100% <br /> Upon qualifying for and receiving Parts A and 13 of Medicare, the City shall not be obligated to <br /> contribute a monthly amount in excess of the monthly premium for the Kaiser S -1 supplement <br /> to Medicare Health Plan for employee and one dependent. In the case where the eligible <br /> employee is deceased, the amount the City is obligated to pay shall be reduced by 1/2 (one- <br /> half). If the spouse remarries, the obligation by the City shall terminate. <br /> 6.3 Dental lnsurance <br /> The City at its expense shall provide management and confidential employees and their eligible <br /> dependents enrollment in the present Ameritas Bankers Life Nebraska dental plan. Plan <br /> benefits shall include a maximum lifetime orthodontia benefit of 51,500. <br /> 6.4 Life Insurance <br /> For the purpose of providing life and accidental death and dismemberment insurance to <br /> management and confidential employees, the City shall provide at its expense coverage equal <br /> to twice the employee's annual salary to a maximum coverage of one hundred thousand dollars <br /> ($100,000). <br /> 6.5 Disability Insurance <br /> 6.5(1) Short -Term Disability <br /> During his/her second through fourth year of City employment, a regular management or <br /> confidential employee on extended medical leave may be eligible for a partial income <br /> continuation benefit. To be eligible for said benefit, the employee's attendance record must be <br /> satisfactory as judged by the employee's department head and City Manager. <br /> If approved, the employee's weekly income continuation benefit will equal 55% of his/her <br /> regular weekly rate of pay to a maximum weekly benefit of $336.00.
