5.3 Dental Insurance. <br /> <br />Management and confidential employees and their eligible <br />dependents shall be offered enrollment in the present <br />Bankers Life Nebraska dental plan to include a maximum <br />lifetime orthodontia benefit of $1,000. <br /> <br />5.4 Life Insurance. <br /> <br />For the purpose of providing life and accidental death and <br />dismemberment insurance to management and confidential <br />employees, the city shall provide coverage equal to twice <br />the employee's annual salary to a maximum coverage of one <br />hundred thousand dollars ($100,000). The total monthly <br />premium for said coverage shall be paid by the city. <br /> <br />5.5 Long-Term Disability Insurance. <br /> <br />The city shall provide management and confidential <br />employees long term disability insurance coverage at no <br />cost to the employee. Effective October 1, 1989 the <br />maximum monthly disability insurance benefit shall be <br />increased from $1,500.00 to $3,000. <br /> <br />5.6 Cafeteria Program <br /> <br />A cafeteria program may be developed and implemented. <br />Details of the program will be presented separately for <br />incorporation into subsequent management plans. <br /> <br />Section 6.
