personal affidavit with the Personnel Officer stating the <br /> cause of the absence. <br /> 15.3 Sick Leave Increments <br /> Charge for sick leave used shall be on the basis of one <br /> (1) hour for each hour used in increments of twenty -four <br /> (24) hours for employees assigned to a 24 -hour duty <br /> schedule; provided, however, that: <br /> (1) If an employee reports for duty before 12:00 Noon on <br /> the second or any subsequent consecutive on -duty day <br /> after an illness with a written statement from a <br /> person licensed to practice medicine or dentistry, <br /> or, <br /> (2) If an employee becomes ill while on the job and must <br /> leave before the end of his /her shift, sick leave <br /> shall be charged for only those hours when the <br /> employee was absent from work. <br /> Section 16. Leaves of Absence <br /> 16.1 Leave of Absence Without Pay <br /> The appointing authority may grant a full -time employee <br /> leave of absence without pay or seniority for not to <br /> exceed three (3) months. No such leave shall be granted <br /> except in writing and upon written request of the employee <br /> setting forth the reason therefor. Upon expiration of a <br /> regularly approved leave the employee shall be reinstated <br /> in the position held at the time leave was granted. <br /> Failure on the part of an employee on leave to report <br /> promptly at its expiration or within a reasonable time <br /> after notice to return to duty shall be cause for <br /> discipline. The Fire Chief may grant a full -time employee <br /> leave of absence without pay for not to exceed one (1) <br /> calendar week. Such leaves shall be reported to the <br /> Personnel Officer. <br /> 16.2 Jury Leave <br /> Every employee of the City who is called or required to <br /> serve as a juror shall be entitled to leave from his /her <br /> duties with the City during the period of such service or <br /> while necessarily being present in court as a result of <br /> such call. The employee shall be paid the difference <br /> between his /her full salary and any payment received by <br /> him /her, except travel pay, for such duty. <br /> - 15 - <br />
