15.3 <br /> <br />15.4 <br /> <br />15.5 <br /> <br />Health and Welfare Trust <br /> <br />During the term of the Memorandum of Understanding, the City, <br />upon request by the PCEA, will meet and confer with the <br />Association regarding establishing a Health and Welfare Trust. <br />Should the parties agree to the establishment of such a Trust, <br />the City's contributions on behalf of the employees' will not <br />exceed the amount contributed by the City toward the employee's <br />current medical, dental and life insurance coverage at the time <br />the Trust was established. <br /> <br />Dental <br /> <br />Effective May 1, 1990, the city shall contribute an amount not to <br />exceed forty eight dollars (48.00) per month per employee to <br />provide dental care benefits under the city's present Ameritas <br />Life Insurance Plan for the individual employee and eligible <br />dependents. <br /> <br />Effective May 1, 1991 said contribution shall increase to an <br />amount not to exceed fifty two dollars ($52.00) per month per <br />employee. <br /> <br />For the purpose of providing aforesaid dental care coverage for <br />eligible regular part-time employees including job share <br />employees who average between twenty (20) and twenty-nine (29) <br />hours per week, the City shall contribute an amount not to exceed <br />fifty percent (50%) of the city's monthly contribution <br />established for full time employees as set forth above. For <br />regular part-time employees including job share employees who <br />average between thirty (30) and thirty-nine (39) hours per week, <br />the City shall contribute an amount not to exceed seventy-five <br />percent (75%) of the city's monthly contribution established for <br />full-time employees as set forth above. <br /> <br />Life Insurance <br /> <br />The City shall contribute an amount necessary to provide $20,000 <br />life and $20,000 accidental death and dismemberment insurance <br />coverage for the individual full-time employee. No later than <br />July 1, 1990 said coverage shall increase to $25,000 life and <br />$25,000 accidental death and dismemberment. <br /> <br />Effective March 1, 1988, employees may purchase, at the <br />employees' own expense and within the limits set by the insurance <br />carrier, additional amounts of life insurance under the existing <br />policy. <br /> <br />- 23 - <br /> <br /> <br />
