Section 11. Leaves of Absence <br /> <br />11.1 <br /> <br />General Leave <br /> <br />Any regular or probationary employee may upon written <br />request to the Department Head and the Personnel <br />Director, be granted by the City Manager a leave of <br />absence without pay, for a period not to exceed three <br />(3) months. The written request must state the reasons <br />for the leave. The Department Head may grant a regular <br />or probationary employee a leave of absence without pay, <br />for a period not to exceed one (1) calendar week. Said <br />leaves shall be reported to the Personnel Director. <br /> <br />During said leave, an employee shall not accrue vacation <br />or sick leave benefits or be eligible for holidays, nor <br />shall such time be considered as service time. <br /> <br />11.2 <br /> <br />Extended Medical Leave <br /> <br />Any regular or probationary employee who is temporarily <br />disabled form work due to a medical condition may be <br />upon written request to the Department Head and the <br />Personnel Director, be granted by the City Manager, a <br />leave of absence without pay for a period not to exceed <br />three (3) months beyond the exhaustion of any accrued <br />sick and vacation leave. With a recommendation by the <br />Department Head to and approval by the Personnel <br />Director, the City Manager may extend the leave for up <br />to an additional thirty (30) days. <br /> <br />The term medical disability includes all temporary <br />medical disabilities including pregnancy, childbirth and <br />associated medical conditions. Total approved paid and <br />unpaid time away from work because of medical disability <br />prior to and after the birth of a child, shall not <br />exceed one hundred twenty (120) days. An employee no <br />longer disabled because of pregnancy, childbirth or <br />associated medical conditions may apply for additional <br />unpaid leave time for the care of the child under the <br />provisions of Section 11.1 above. <br /> <br />Such request for medical leave will be considered only <br />upon provision of a physician's statement attesting to <br />said disability. <br /> <br />During extended medical leave, an employee shall not <br />accrue vacation or sick leave benefits or be eligible <br />for holidays, nor shall such time be considered as <br />service time. <br /> <br />Any employee returning from medical leave shall be <br />required to provide a physician's release to return <br />to duty. <br /> <br />Memorandum of Understanding <br />Page 11 <br /> <br /> <br />
