5.3 <br /> <br />5.4 <br /> <br />5.5 <br /> <br />Upon qualifying for and receiving Parts A and B of Medicare, <br />the city shall not be obligated to contribute a monthly amount <br />in excess of the monthly premium for the Kaiser S-1 supplement <br />to Medicare Health Plan for employee and one dependent. In <br />the case where the eligible employee is deceased, the amount <br />the city is obligated to pay shall be reduced by 1/2 (one- <br />half). If the spouse remarries, the obligation by the city <br />shall terminate. <br /> <br />Dental Insurance <br /> <br />Management and confidential employees and their eligible <br />dependents shall be offered enrollment in the present Ameritis <br />Bankers Life Nebraska dental plan to include a maximum <br />lifetime orthodontia benefit of $1,500. <br /> <br />Life Insurance. <br /> <br />For the purpose of providing life and accidental death and <br />dismemberment insurance to management and confidential <br />employees, the city shall provide coverage equal to twice the <br />employee's annual salary to a maximum coverage of one hundred <br />thousand dollars ($100,000). The total monthly premium for <br />said coverage shall be paid by the city. <br /> <br />Lg~a-Term Disability Insurance. <br /> <br />The city shall provide management and confidential employees <br />long term disability insurance coverage at no cost to the <br />employee.
