Section 13. Leaves of ~bsence <br /> <br />13.1 Family Leave <br /> <br />Employees shall be eligible for family leave in accordance with the <br />State "Family Rights Act of 1991." <br /> <br />13.2 <br /> <br />Personal Leave <br /> <br />Any regular or probationary employee may upon written request to <br />his/her Department Head and the Personnel Director, be granted by <br />the city Manager a leave of absence without pay for unique personal <br />reasons for a period not to exceed three (3) months. The written <br />request must state the reasons for the leave. The Department Head <br />may grant a regular or probationary employee leave of absence <br />without pay for a period not to exceed one (1) calendar week. Said <br />leaves shall be reported to the Personnel Director. In addition, <br />the employee's leave balances shall be taken into account in the <br />City Manager's consideration of the request. The City Manager will <br />also consider the City's ability to get the work tasks accomplished <br />in the employee's absence. <br /> <br />During said leave, an employee shall not accrue vacation or sick <br />leave benefits or be eligible for holidays, nor shall such time be <br />considered as service time. <br /> <br />13.3 <br /> <br />E~$ended Medical Leave <br /> <br />Any regular or probationary employee who is temporarily disabled <br />from work due to a medical condition may, upon written request to <br />the Department Head and the Personnel Director, be granted by the <br />City Manager, a leave of absence without pay for a period not to <br />exceed three (3) months beyond the exhaustion of any accrued sick <br />and vacation leave. With a recommendation by the Department Head <br />to and approval by the Personnel Director, the City Manager may <br />extend the leave for up to an additional thirty (30) days. <br /> <br />The term medical disability includes all temporary medical <br />disabilities including pregnancy, childbirth and associated medical <br />conditions. Total approved paid and unpaid time away from work <br />because of medical disability prior to and after the birth of a <br />child, shall not exoeed one hundred twenty (120) days. <br /> <br />Such request for medical leave will be considered only upon <br />provision of a physician's statement attesting to said disability. <br /> <br />During extended medical leave, an employee shall not accrue <br />vacation or sick leave benefits or be eligible for holidays, nor <br />shall such time be considered as service time. Any employee <br />returning from medical leave shall be required to provide a <br />physician's release to return to duty. <br /> <br />- 16 - <br /> <br /> <br />
