This provision does not apply if the family sickness or <br /> disability or the employee's bereavement occurs while the <br /> employee is on vacation leave. <br /> <br /> An employee eligible for temporary disability payments may use <br /> accumulated sick leave in order to maintain his/her regular income; <br /> provided, however, that the employee shall be allowed a credit <br /> against sick leave to the extent that temporary disabilitypayments <br /> are retained by the City. <br /> <br /> In order to receive compensation while absent on sick leave the <br /> employee shall notify his/her immediate supervisor or the Person-- <br /> nel Director prior to or within one hour after the time set for <br /> beginning his/her daily duties or as may be specified by the <br /> Department Head. When absence is for more than eight (8) hours, <br /> the employee may be required to file a physician's certificate or <br /> a personal affidavit with the Personnel Director stating the cause <br /> of the absence. <br /> <br />Section 13, ~eaves of Absence <br /> <br />13.1 Family Leave <br /> <br /> Employees shall be eligible for family leave in accordance with the <br /> State "Family Rights Act of 1991," and the "Federal Family and <br /> Medical Leave Act 1993.n <br /> <br />13.2 personal Leave <br /> <br /> Any regular or probationary employee may upon written request to <br /> his/her Department Head and the Personnel Director, be granted by <br /> the City Manager a leave of absence without pay for unique personal <br /> reasons for a period not to exceed three (3) months. The written <br /> request must state the reasons for the leave. The Department Head <br /> may grant a regular or probationary employee leave of absence <br /> without pay for a period not to exceed one (1) calendar week. Said <br /> leaves shall be reported to the Personnel Director. In addition, <br /> the employee's leave balances shall be taken into account in the <br /> City Manager's consideration of the request. The City Manager will <br /> also consider the City's ability to get the work tasks accomplished <br /> in the employee's absence. <br /> <br /> During said leave, an employee shall not accrue vacation or sick <br /> leave benefits or be eligible for holidays, nor shall such time be <br /> considered as service time. <br /> <br />13.3 Extended Medical Leave <br /> <br /> Any regular or probationary employee who is temporarily disabled <br /> from work due to a medical condition may, upon written request to <br /> the Department Head and the Personnel Director, be granted by the <br /> City Manager, a leave of absence without pay for a period not to <br /> <br /> - 16 - <br /> <br /> <br />
