Life. Accidental Dead Land Dismemberment Insurance <br />As soon as practical but no later than October 1, 1990, the City shall increase the present employee <br />$10,000 life and $10,000 accidental death and dismemberment insurance coverage to $25,000 life <br />and $25,000 accidental death and dismemberment insurance coverage. The monthly premium for <br />life insurance coverage shall he paid by the City. Employee may also participate in the City's <br />supplemental life insurance program at their expense. <br />1 one - Term Disability <br />The City shall pay to the Union the sum of fifteen dollars ($15.00) as the monthly premium of each <br />employee enrolled in the union's long term disability plan. Said payments shall be made in arrears <br />monthly. Should the monthly premium of the Union's long term disability plan drop below fifteen <br />dollars per month, the dollar difference shall be added one time to the rate of pay set forth in <br />Section 5.1 of this agreement <br />Extended Benefits <br />The City will consider voluntary proposals from the Union's membership to utilize their benefits <br />to extend the benefits of an individual on protracted medical leave. <br />E. Sirk Leave <br />Sick heave Accrual <br />During his /her one year probationary period each new full -time employee shall earn sick leave at <br />the rate of eleven and two - tenths (11.2) hours for each month of service. Upon successfully <br />completing his /her one year probationary period each new employee shall receive five hundred <br />thirty -seven and six - tenths (537.6) hours sick leave credit in addition to that earned during <br />probation. Upon completion of five (5) years service every employee shall thereafter accumulate <br />sick leave at a rate of eleven and two- tenths (11.2) hours per month to a total of 2,016. For each <br />month an employee maintains a sick leave balance of 2,016 hours or more, he /she shall be credited <br />with an additional 11.2 hours of sick leave. In no event shall an employee accrue more than 2,912 <br />hours of sick leave. An employee may be allowed a sick leave deficit of thirty -three and six - tenths <br />(33.6) hours beyond that actually earned. Employees on leave of absence with pay in lieu of <br />temporary disability payments pursuant to Section 4850 of the Labor Code shall accumulate sick <br />leave during said leave of absence, in accordance with the provisions of this section. <br />Sick Leave Usagx <br />Sick leave with pay shall be granted to all full -time employees. Sick leave shall not be considered <br />a right which an employee may use in his /her discretion, but shall be allowed only as follows: <br />(t) <br />In cases of necessity and actual person sickness or disability of the employee. <br />31 <br />
