6.4 Life Insurance. <br /> <br /> For the purpose of providing life and accidental death and dismemberment insurance to <br /> management and confidential employees. the City shall provide at its expense coverage <br /> equal to twice the employee's annual salary to a maximum coverage of one hundred <br /> thousand dollars ($100,000). <br /> <br />6.5 Disability Insurance <br /> <br />6.5(1) Short-Term DisabiliW <br /> <br /> During his/her second through fourth year of City employment, a regular management or <br /> confidential employee on extended medical leave may be eligible for a partial income <br /> continuation benefit. To be eligible for said benefit, the employee's attendance record <br /> must be satisfactory as judged by the employee's department head and City Manager. <br /> <br /> If approved. the employee's weekly income continuation benefit will equal 55% of his/her <br /> regular weekly rate of pay to a maximum weekly benefit of $336.00. The schedule of <br /> benefit payments shall be as follows: <br /> <br /> Employee's Year of Employment Maximum Weeks of Benefit <br /> <br /> Second Year Six Weeks <br /> Third Year Four Weeks <br /> Fourth Year Two Weeks <br /> <br /> The City, at its discretion, may choose to convert the above income continuation benefit <br /> from a self-insured benefit to an insured benefit. <br /> <br /> 6.5(2) Long-Term Disability Insurance. <br /> <br /> The City shall provide, at its expense. management and confidential employees long-term <br /> disability insurance coverage. The maximum monthly benefit will be $4,000.00 with a <br /> 60-day waiting period. <br /> <br /> The City shall seek to amend its present long-term disability insurance policy to include <br /> pro-rated disability insurance benefits for regular part time confidential employees. <br /> Implementation of said benefits is subject to approval of the insurance carrier. <br /> <br /> 6.6 Vision Care <br /> <br /> Effective September 1, 1997 management and confidential employees and their eligible <br /> dependents enrolled in either of the City's Foundation or Blue Cross health plans shall be <br /> enrolled in the City's vision care plan. The premium for said vision care plan for both the <br /> employee and their eligible dependents shall be paid by the City. <br /> <br /> 7 <br /> <br /> <br />
