Ctass Cwtth Special Certifmcates, issued pursuant to CVC Section 2512, 12517, 12519, 12520, 12523, or12523.50 <br /> 4. Any driver of a passenger vehicle having a seating c, apa~ of not more than 10 persons, including the driver, <br /> operated for c~ by a charter-party career of passengers <br /> _ 5. Passenger stage corporation with a certificate of public convene or permit issued by PUC. <br /> <br />All owner operators am enrolled by PUC. <br /> <br />WHO MAY ENROLL <br /> <br />Employers may at their discretion, volunteer to enroll drivers in ltm interest of driver safety. <br /> <br />ENROlLiNG I)IllVERS <br /> <br />Employers will receive token copies of the Employer Pull Notice Enrollment or Deletions of Drivers, INF 1100. These forms <br />are to be used either for enrolling or deleting drivers (Do not combine enrolling or deleting of drivers on the same form.) <br /> <br />OUT OF SLATE LICENSEI) DRNERS <br /> <br />Employers with drivers who am licensed in another state, will complete the Out of State Commercial Driver License Em- <br />ployer Pull Notice Program, INF 1102. An index number (*X number") will be assigned in place of a California DL number for <br />tracking purposes. <br /> <br />MAGNETIC TAPE <br /> <br />The department offers employers the option of enrolling and deleting ddvers by computer magnetic tape. Please call the <br />Employer Pull Notice Unit at (916) 657-6346 for instructions and specifications. <br /> <br />DELETING DRIVERS <br /> <br />Immediately upon termination of employment, employers must notify the Department to delete the terminated driver by <br />completing the EPN Enrollment/Deletion form, INF 1100. Employers with ddvers who are licensed in another state am <br />deleted by using the assigned index number ("x" number.) <br /> <br /> PLEASE NOTE: Deletions not promptly reported may result in unnecessary driver records being sent end the subse- <br /> quent billing for those records. These fees are non-refundable. <br /> <br />EPN REPORTS <br /> <br />Employers are required to receive ulxiated employee driver records periodically. The EPN Program automatically generates <br />a ddver's current record as follows: <br /> <br /> 1. Upon enrollment of drivers in the EPN <br /> 2. Semiannually if the employer has less than 500 drivers <br /> 3. Annually if the employer has more than 500 drivers <br /> 4. When a driver's has any of the actions added to his/her driver recoffi as follows <br /> A. Convictions <br /> B. Failures To Appear <br /> C. Accidents <br /> D. DL Suspensions <br /> E. DL Revocations <br /> F.
