EXHIBIT B <br />CONDITIONS OF APPROVAL <br />PCUP-146, Frank Fiala, Frank Fiala Roofing, Inc. <br />1228 Quarry Lane, Unit C <br />December 14, 2005 <br />1. 'I-'he location acid operation oi-the proposed use shall conform substantially to <br />F_xhibit A the site plan, floor plans, acid details of operation, dated "Received, <br />October 26, 2005," on file with the Plaruiing Department, except as modified by the <br />following conditions of approval.. Minor changes to the approved operation and <br />schedule may be approved by the Planning Director if found to be in substantial <br />compliance with the proposal. <br />2_ If rnodif3cations or expansion of the use beyond what is shown in Exhibit A, dated <br />"Received October 26, 2005," are desired, prior City review and approval is req uired_ <br />Such modification may be approved by the Planning Director if found to be in <br />substantial conformance with the approval. The Planning Director may also refer the <br />matter to the Planning Commission if the proposed changes would be significant. <br />3. All Conditions of Approval of PUD-80-O 7 shall remain in full force and effect. <br />4. All Conditions of Approval of Z-82-128 Design Reviews shall remain in frill force <br />and effect. <br />5_ The hours for warehousing/outdoor storage activities shall be from 7:00 a_m_ to <br />7:00 p_ni_ <br />6_ Prior to operation, the applicant sha71 provide a Hazardous Matvri al Management Plan <br />to the Fire Marshal for review and approval, if necessary. <br />7. Prior to operation, the applicant shall revise the site plan to show that a 20-foot wide <br />opening is provided at the chain link gates. If a new chain link gates are needed, the <br />applicant shall submit the gate detail to the Planning Director for review and approval <br />prior to installation. <br />8_ Prior to operation, the applicant shall Sri stall Knox box on the chain link gates. "l'he <br />details and specifications of the Knox box shall be reviewed and approved by the Pire <br />Marshal. <br />9. Prior to operation, the applicant shall provide a storm water run off management plan <br />stating that the all outdoor warehousing materials, equipments, and vehicles would. be <br />covered by weatherproof tarps. If it is found that dump trucks are returned from job <br />sites fully loaded on a regular basis, ai the discretion of the Planning Director, the <br />applicant is required to install/construct a structure to cover the area for dump trucks <br />tar kettle(s~ parking.
