(end) <br />EXHIBIT B <br />CONDITIONS OF APPROVAL <br />PCUP -138, Jennifer Sheng/Little Ivy League <br />2340 Santa Rita Road, Suite 6 <br />April 27, 2005 <br />1. The location and operation of the proposed use shall conform substantially to Exhibit A (the <br />site plan, floor plans, and details of operation), dated "Received, April 13, 2005 on file with <br />the Planning Department, except as modified by the following conditions of approval. Minor <br />changes to the approved operation and schedule may be approved by the Planning Director if <br />found to be in substantial compliance with the proposal. <br />2. If additional hours of operation beyond what was included in the applicant's statement of <br />operation, dated "Received, April 13, 2005 on file in the Planning Department, are desired, <br />the applicant shall obtain approval from the State of California Department of Social <br />Services prior to City review and approval. <br />3. Additional hours and activities beyond what was stated in the applicant's statement of <br />operation, dated "Received April 13, 2005 on file in the Planning Department may be <br />approved by the Planning Director if found to be in substantial conformance with the <br />approval. The Planning Director may also refer the matter to the Planning Commission if the <br />4. proposed changes would be significant. <br />5. If operation of this use permit results in conflicts pertaining to parking, noise, <br />traffic /circulation, or other factors, at the discretion of the Planning Director, this conditional <br />use permit may be submitted to the Planning Commission for their subsequent review at a <br />public hearing. If necessary, the Planning Commission may modify or add conditions of <br />approval to mitigate such impacts, or may revoke the said conditional use permit approval. <br />6. Prior to operation, the applicant shall contact the Building Inspection Department and the <br />Fire Marshal to conform to Building Code and Fire Code requirements, if applicable. The <br />applicant shall obtain a building permit prior to commencement of any work, if required. No <br />changes to the exterior of the building shall be made without prior approval from the <br />Planning Department. <br />7. The applicant shall obtain a City business license as soon as this use permit approval <br />becomes effective. <br />1 <br />
