A-. <br />j nd) <br />-EXHIBIT B <br />CONDITIONS OF APt'12O <br />PCUP -138, Jennifer Slteng /Little Ivy League <br />2340 Santa Rita Road, Suite 6 <br />April 17, 2005 <br />The location and operation of the proposed use shall conform substantially to <br />>✓xhibit A (the site plan, floor plans, and details of dated <br />"Received, April 13, 2005 on file with the Planning Department, except as <br />modified by the following conditions of approval_ Minor changes to the <br />approved operation and schedule may Be approved by the Planning Director if <br />found to be in substantial compliance with the proposal_ <br />If additional hours of beyond what was included in the applicant's <br />statement of operation, dated "Received, April 13, 2005, on file in the <br />Planning Department, are desired, the applicant shall obtain approval from the <br />State of California Department of Social Services prior to City review and <br />approval_ <br />3. Additional hours and activities beyond what was stated in the applicant's <br />statement of operation, dated "Received April 13, 2005", on file in the <br />Planning Department may be approved by the Planning Director if to be <br />in substantial conformance with the approval_ The Planning Director may also <br />refer the rnatter to the Planning Commission if proposed changes would be <br />significant_ <br />IF operation of use permit results in conflicts pertaining to parking, noise, <br />traffic /circulation, or other factors, at the discretion of the Planning Director, <br />this conditional use permit may be submitted to the Planning Commission for <br />their subsequent review at a public hearing. If necessary, the Planning <br />Commission may modify or add conditions of approval to mitigate such <br />impacts, or niay revoke the said conditional use permit approval_ <br />5. Prior to operation., the applicant shall contact the Building Inspection <br />Department and the Fire Marshal to conform to Building Code and Fire Code <br />requirements, if applicable. "fhe applicant shall obtain a building permit prior <br />LCD commencement of any work, if No changes to the exterior of the <br />building shall be made without prior approval from the Planning Department_ <br />The applicant shall obtain a City business license as soon as this use perrnit <br />approval becomes effective. <br />1 <br />
