If approved, the employee's weekly income continuation benefit will equal 55% of <br /> his /her regular weekly rate of pay to a maximum weekly benefit of $336.00. The <br /> schedule of benefit payments shall be as follows: <br /> Employee's Year of Employment Maximum Weeks of Benefit <br /> Second Year Six Weeks <br /> Third Year Four Weeks <br /> Fourth Year Two Weeks <br /> The City, at its discretion, may choose to convert the above income continuation <br /> benefit from a self - insured benefit to an insured benefit. <br /> An additional voluntary short-term disability plan will be made available at the <br /> employee's cost. <br /> 5.5(2) Long-Term Disability Insurance <br /> Management: <br /> The City shall provide, at the employee's expense, long -term disability insurance <br /> coverage for management employees. The maximum monthly benefit will be $6,500 <br /> with a 60 -day waiting period. <br /> Confidential: <br /> The City shall provide, at its expense, long -term disability insurance coverage for <br /> confidential employees. The maximum monthly benefit will be $4,000 with a 60 -day <br /> waiting period. <br /> 5.6 Vision Care <br /> Effective September 1, 1997 management and confidential employees and their <br /> eligible dependents enrolled in either of the City's Health Net health plans shall be <br /> enrolled in the City's Vision Care plan. The premium for the Vision Care plan for both <br /> the employee and eligible dependents shall be paid by the City. The premium paid by <br /> the City for regular part-time management and confidential employees shall be <br /> prorated. <br /> Page 8 of 31 <br />
