V. REQUIRED INFORMATION <br /> <br />a) Proposal preparation <br />All responses to this RFP but must be made in accordance with the specifications as set forth <br />herein. Failure to adhere to any specifications contained herein may be cause to reject your <br />proposal. <br /> <br />b) Ability to Perform <br /> <br />As part of your proposal, you must present satisfactory evidence indicating your ability to meet <br />the scope of work detailed in this RFP. Further, you must provide adequate pricing and service <br />information to assure that the fees for services and products are clearly described. To this end, <br />your proposal must include the following information: <br /> <br />If appropriate, the names, business address and telephone numbers of your company's <br />officers, directors, and associates and the names and addresses of any parent of subsidiary of <br />your company. The proposal must describe the nature of the work and the line of authority <br />of these individuals and/or companies as they relate to this project. <br /> <br />A listing of your experience related to work included in this RFP. Contractor must have at <br />least 5 years &business experience and have successfully completed at least 1 year of <br />contract services for a governmental agency within California. <br /> <br />3. A minimum of three related business references, including their names, addresses, and phone <br /> numbers, plus a description &the type of work you performed for them. <br /> <br />A detailed work plan describing your approach to managing and coordinating this project. <br />The description should, at a minimum, include all tasks listed in Section II, "Scope of Work." <br />In addition, you should include any steps/tasks not included in the "Scope of Work" that you <br />think would materially affect the quality of the project. The quality and thoroughness of the <br />proposer's work plan is an evaluation criterion. <br /> <br />5. The resume(s) of all individual supervisor(s) performing work on the project including <br /> identification of the project manager. <br /> <br />A description of any plan to subcontract any portion of the work to an individual or <br />organization other than yourself. The description must include the type and length of work, <br />name of the subcontractor(s), and the names, titles, and qualifications of all individuals <br />performing project work for the subcontractor. <br /> <br />A detailed fee schedule including your preferred invoicing procedures. The City anticipates <br />a "not to exceed" fee for this project. An open-ended hourly-based fee proposal will be <br />determined to be incomplete. <br /> <br /> <br />
