The proposed use shall conform substantially to the proj ect <br />description, written narrative, site plan, and floor plan, <br />Exhibit "A", dated "Received June 16, 1995" on file with the <br />Planning Department, except as modified by these conditions. <br /> <br />2. The project proposal shall not include the establishment of <br />cooking classes as outlined in the project narrative. <br /> <br />3. The applicant shall obtain all building and other applicable <br />City permits for the use and structure prior to the commencement <br />of the use. <br /> <br />4. Childcare shall be provided on site for the use of members only <br />and shall be made available only when the child's guardian is <br />present on site at the health club. <br /> <br />5 . <br /> <br />The applicant shall obtain all necessary permits from the <br />including any requirements from the Department of <br />Services, for the child care facility and County prior <br />child care operation requiring a permit. <br /> <br />State, <br />Social <br />to any <br /> <br />6. The hours of operation of the health and fitness center shall be <br />6:00 a.m. to 11:00 p.m., Monday through Friday and 7:00 a.m. to <br />7:00 p.m. Saturday and Sunday. Additional hours of operation <br />may be submitted for the review and approval by the Planning <br />Director. <br /> <br />7. If any changes in the health club operations or additional <br />activities beyond that proposed by the written narrative are <br />desired, prior to City review an approval is required. Requests <br />for modification of the use permit shall be submitted to the <br />Planning Director. Said changes may require a modification of <br />the use permit or, upon determination of the Planning Director, <br />a new use permit may be required. <br /> <br />8. The applicant shall install a noise muffling wall along the <br />northern and southern walls surrounding the aerobics areas and <br />the eastern building wall at the rear of the suite to attenuate <br />noise from the proposed fitness/aerobics classes. The wall <br />shall be designed to achieve an STC rating of 45-50, subject to <br />the satisfaction of the Chief Building Official. <br />
