The proposed use shall conform substantially to the proj ect description, written narrative, site plan, and floor plan, Exhibit "A", dated "Received June 16, 1995" on file with the Planning Department, except as modified by these conditions. 2. The project proposal shall not include the establishment of cooking classes as outlined in the project narrative. 3. The applicant shall obtain all building and other applicable City permits for the use and structure prior to the commencement of the use. 4. Childcare shall be provided on site for the use of members only and shall be made available only when the child's guardian is present on site at the health club. 5 . Additional hours of operation may be submitted for the review and approval by the Planning Director. 7. If any changes in the health club operations or additional activities beyond that proposed by the written narrative are desired, prior to City review an approval is required. Requests for modification of the use permit shall be submitted to the Planning Director. Said changes may require a modification of the use permit or, upon determination of the Planning Director, a new use permit may be required. 8. The applicant shall install a noise muffling wall along the northern and southern walls surrounding the aerobics areas and the eastern building wall at the rear of the suite to attenuate noise from the proposed fitness/aerobics classes. The wall shall be designed to achieve an STC rating of 45-50, subject to the satisfaction of the Chief Building Official.
