<br />Resolution No. PC-96-71 <br />Page 4 <br /> <br />EXHIBIT "B" <br />Conditions of Approval <br />Holoz Lazer Tag <br />Case UP-96-46 <br /> <br />I. The proposed use shall conform substantially to the project description, written narrative, <br />site plan, and floor plan, Exhibit "A," dated "Received July 19, 1996," on file with the <br />Planning Department, except as modified by these conditions. <br /> <br />2. If the operation of the use results in major conflicts pertaining to automobile/bicyc1e <br />parking, noise, traffic, or other impacts, at the discretion of the Planning Director, this <br />conditional use permit may be referred to the Planning Commission for subsequent <br />review at a public hearing. If necessary, the Commission may modify or add conditions <br />of approval to mitigate such impacts, or may revoke the said conditional use permit. <br /> <br />3. If additional activities beyond that proposed in the project description, written narrative, <br />site plan, or floor plan, dated "Received July 19, 1996," on file with the Planning <br />Department are desired, prior City review and approval are required. Such modifications <br />shall be submitted to the Planning Director, who shall refer the matter to the Planning <br />Commission if determined to be substantial. <br /> <br />4. The hours of operation shall be 10:00 a.m. to 10:00 p.m. Sunday through Thursday and <br />10:00 a.m. to 12:00 midnight Fridays and Saturdays. Minor changes to the hours of <br />operation may be submitted for the review and approval by the Planning Director. <br /> <br />5. Employees shall park in the rear parking lot area along Stoneridge Mall Road during <br />business hours. <br /> <br />6. A security guard shall be provided on-site during business hours (except for school days, <br />when the security guard need only be on duty from 3 :00 p.m. to closing) to supervise and <br />control the activities of the facility's patrons both inside the building and on site in the <br />surrounding parking lot. In particular, the security guard shall monitor the activities of <br />customers in the rear parking lot area along Stoneridge Road and Spring dale Avenue. <br />After six months of operation, the applicant may request that this requirement be <br />eliminated or restricted to evening hours. Such a request shall be reviewed by the <br />Planning Director and Police Chief, who may modify this requirement at their discretion <br />if determined that no problems within the facility or in tl1e parking lot or surrounding <br />neighborhood have occurred. <br /> <br />7. The use shall be conducted entirely within the building at all times, and the doorways to <br />the recreational facility shall remain closed except for entering and exiting the site or for <br />the purpose of loading/unloading activities. <br />
