<br />Resolution No. PC-96-16 <br />Page 5 <br /> <br />EXHIBIT "B" <br />PUD-90-07-2M <br />PLANNING COMMISSION <br />CONDITIONS OF APPROVAL <br /> <br />1) The design and operation of the assisted living facility shall be substantially similar to <br />the site plan, landscape plan, architectural plans, and colorboards, Exhibit A, dated <br />"Received January 18, 1996," and applicants' narratives, dated November 27,1995 and <br />January 23,1996, on file with the Planning Department, except as modified herein. <br /> <br />2) The developer shall provide a private gathering room through the use of movable <br />partitions, or other acceptable means, in the dining room area for use by the residents <br />and their family members/friends on special occasions. Additionally, a restroom shall be <br />added to the westernmost lounge area for use by the residents. These changes shall be <br />submitted for the review and approval by the Planning Director on the plans submitted <br />for the issuance of a building permit. <br /> <br />3) Residents of this facility shall not be permitted to keep cars or other vehicles on the <br />premises of the facility nor the adjacent office parking lot. This provision shall be <br />diligently enforced by the operator of this facility. <br /> <br />4) The operator of this facility shall maintain at all times a van pool service sufficient to <br />ensure that use of the City's Dial-A-Ride service will not be necessary for residents of <br />the facility. If necessary to adequately serve the residents, the applicant shall add a <br />second van for the facility. <br /> <br />5) Any proposed change to the operation and use of this facility shall be submitted for the <br />review and approval by the Planning Director, and at his/her discretion, noticed to the <br />neighbors, prior to his/her taking an appropriate action, or referred to the Planning <br />Commission for its review. <br /> <br />6) A final landscape plan and irrigation plan shall be submitted to and approved by <br />Planning Director as part of the building permit plan set prior to issuance of an on-site <br />permit. Said landscape plan shall be consistent with the approved landscape plan plus <br />any conditions of approval, and shall be detailed in terms of species, location, size, <br />quantities, and spacing. The following items shall be incorporated on the plan: <br /> <br />a) The landscape plan shall be modified to include podocarpus or similar tall <br />shrub/ small accent tree in the area between the employee lounge and the <br />westernmost parking lot in order to help soften the appearance of the <br />parapet screen wall around the equipment well. <br />
