Patients in this category experience negative health outcomes from their <br />substance use, as well as continued exposure to triggering situations (i.e., others using around <br />them, availability of substances, etc). Some examples of the services provided during visits <br />include: medical treatment for wounds, assessment of current medical conditions, including <br />chronic health concerns, referrals/assistance locating housing, or being placed on waiting lists, <br />disability paperwork, referrals for specialty care, lab orders, medications provided, medical <br />coverage and applying for coverage, mental health and Substance Use Disorder referrals, and <br />transportation vouchers. In FY 2023/24, $18,550 was expended for this service, and a total of <br />16 unique patients were served, ages 28 to 69. <br /> <br />Axis Community Health is no longer providing this service. <br /> <br />DISCUSSION <br />Staff recommend allocating $100,000 from the City’s Opioid Settlement Funds to support La <br />Familia’s Substance Use Disorder Facility to be constructed to serve the Tri-Valley region. The <br /> proposed project aligns with the allowable uses of opioid settlement funds to support <br />substance use disorder facilities. In July 2023, La Familia was awarded $7.6 million in <br />Behavioral Health Continuum Infrastructure Program (BHCIP) funding from the State for the <br />acquisition, renovation, and construction of the facility. The center will provide licensed, high- <br />quality care in a therapeutic environment and expand the County's continuum of services, as <br />follows: <br /> <br />Table 2. Completed assessments include: <br />• 3D imaging of the property <br />• Septic and water system analyses <br />• Structural review of the private bridge on the property and flood zone analysis to inform <br />planning. <br /> <br />Located within unincorporated Alameda County, the facility will undergo an entitlement process <br />in coordination with County staff. Use permits are expected to be finalized and submitted by <br />November, with development plans completed by February. These steps will set the stage for <br />all predevelopment activities to conclude by February 2026, followed by construction, with <br />completion anticipated by May 2027. The facility is expected to officially open and initiate <br />operations by July 2027. <br /> <br />Operational support will be anchored by Alameda County Behavioral Health through a 30-year <br />Page 19 of 264
