Resolution No. PC-97-44 Page 3 EXHIBIT "B" Conditions of Approval Case UP-97-22 Rickie Slominski - One Park Avenue 1. The proposed use shall conform substantially to the site plan, floor plan, written narrative, and related materials, Exhibit A (dated "Received April 14, 1997"), on file with the Planning Department, except as modified by these conditions. 2. The hours of operation of the therapeutic massage services shall be Thursday and Saturday from 8:00 a.m. to 8:00 p.m. 3. If additional hours or activities beyond that proposed are desired (e.g., additional massage therapists, extended hours, etc.), prior City review and approval is required. Details of such activities shall be submitted to the Planning Director, who may approve the modification if it is judged to be minor, or may refer the matter to the Planning Commission for a public hearing if judged to be substantial. 4. If operation of this use results in conflicts pertaining to parking availability, noise, traffic/circulation, or other nuisances, the Planning Director may modify or add conditions of approval, or submit the matter to the Planning Commission for its subsequent review at a public hearing. If necessary, the Commission may modify or add conditions of approval to mitigate such impacts, or may revoke said use permit. 5. This use permit is not transferable to another address. A new application must be made if the applicants relocate to another site. 6. No business signs are approved as part of this application. Business signs shall conform to the existing approved sign program for this building. 7. Prior to operation, the applicant shall obtain all applicable Police and Fire Department approvals. {end}
