<br />Resolution No. PC-97-44 <br />Page 3 <br /> <br />EXHIBIT "B" <br />Conditions of Approval <br />Case UP-97-22 <br />Rickie Slominski - One Park Avenue <br /> <br />1. The proposed use shall conform substantially to the site plan, floor plan, written narrative, <br />and related materials, Exhibit A (dated "Received April 14, 1997"), on file with the Planning <br />Department, except as modified by these conditions. <br /> <br />2. The hours of operation of the therapeutic massage services shall be Thursday and Saturday <br />from 8:00 a.m. to 8:00 p.m. <br /> <br />3. If additional hours or activities beyond that proposed are desired (e.g., additional massage <br />therapists, extended hours, etc.), prior City review and approval is required. Details of such <br />activities shall be submitted to the Planning Director, who may approve the modification if it <br />is judged to be minor, or may refer the matter to the Planning Commission for a public <br />hearing if judged to be substantial. <br /> <br />4. If operation of this use results in conflicts pertaining to parking availability, noise, <br />traffic/circulation, or other nuisances, the Planning Director may modify or add conditions of <br />approval, or submit the matter to the Planning Commission for its subsequent review at a <br />public hearing. If necessary, the Commission may modify or add conditions of approval to <br />mitigate such impacts, or may revoke said use permit. <br /> <br />5. This use permit is not transferable to another address. A new application must be made if the <br />applicants relocate to another site. <br /> <br />6. No business signs are approved as part of this application. Business signs shall conform to <br />the existing approved sign program for this building. <br /> <br />7. Prior to operation, the applicant shall obtain all applicable Police and Fire Department <br />approvals. <br /> <br />{end} <br />
