City of Pleasanton | Cross-ConnectionControlPlan Project No. PWD 24.202 | September 4,2024 7 <br />Task 2 Deliverables <br />x West Yost will electronically submit a Draft, Final, and Updated Plan to the City in PDF format. <br />x West Yost will submit all original files related to this task to the City on a thumb drive. Original files <br />can be used by the City to update the Plan when required. <br />Task 2 Schedule <br />x Draft Plan: No later than May 2025. <br />x Final Plan: Four (4) weeks from the receipt of City comments. <br />x Updated Plan: Two (2) weeks from the receipt of SWRCB comments. <br />Task 3. Hazard Assessments <br />West Yost’sSpecialistswill advise the City on the hazard assessment approach and account type selection, <br />and will conduct hazard assessments and update the City’s database based on the assessments’ findings. <br />Assessments will be conducted in compliance with the CCCPH Chapter 3, Article 2, Section 3.2.1, and the <br />review conducted in Task 1. Assessments of high hazard premises will be prioritized during the first year. <br />Assessments of low hazard premises with be conducted in the second year if requested by the City as <br />recommended in Task 1. <br />At this time, the number of parcels that require a hazard assessment is not known. West Yost’s project <br />budget includes a lump sum price per Parcelfor reference. When the total number of parcelsthat require <br />a site hazard assessment is assessed during Task 1 and Task 2, a budget augmentation, based on the lump <br />sum price per parcel and the total number of parcels, will be recommended. <br />Task 3 Assumptions <br />x West Yost will use a modified City assessment form to include Specialist name, certification number, <br />and signature. <br />x Meter account number will be used to track data obtained during the assessments. <br />x End-users do not need to be present during the assessment unless entry to private area(s) of a <br />premise is required or if a backflow enclosure or bag is locked. <br />Task 3 Deliverables <br />x Assessment form in PDF format within one week of a site assessment. <br />x Updated database will be provided at the completion of the site assessments. <br />Task 4. As-Needed Services <br />West Yost will assist the City with additional as-needed cross-connection control program services when <br />requested by the City. Additional tasks may include assistance services to conduct public workshop(s) and <br />outreach and attend City Council meetings to describe the requirements of the CCCPH, the City’s Plan, <br />and updated Program or revising the Plan based on SWRCB revisions to the adopted CCCPH. <br />West Yost will not start work under this Task until a specific request and a notice to proceed with an <br />approved budget is received from the City. <br />EXHIBIT B <br />Docusign Envelope ID: A449BB28-7A6A-4FA5-96EA-B9D2D5B83889
