Exhibit A <br />SCOPE OF WORK <br /> <br />Ascertain Forensics at Redwood Toxicology Laboratory, Inc. (Ascertain Forensics at RTL) proposes to <br />provide the following services to the City of Pleasanton. <br /> <br />1. Ascertain Forensics at RTL is currently accredited by American Board of Forensics Toxicology <br />and approved by the California Department of Public Health in compliance with California <br />Administrative Code Title 17 to perform Forensic Alcohol Analysis. Ascertain Forensics at RTL’s <br />toxicology staff currently has six (6) forensic alcohol analysts. <br /> <br />2. Ascertain Forensics at RTL will provide kits for the collection of blood or urine samples for the <br />purpose of analysis for alcohol and/or drugs. <br /> <br />a) Ascertain Forensics at RTL will routinely provide the City of Pleasanton with supplies <br />necessary to collect and ensure proper chain-of-custody for samples. These supplies <br />include collection tubes (vacutainer type) for blood that have a FAS seal from vial to <br />stopper allowing for identification of the proper approved collection vial, non-alcoholic <br />disinfectant wipes and tamper-proof transfer bags. A representative number of tubes <br />from each lot supplied have undergone testing for any contamination from the <br />manufacturer to ensure accurate analytical results for alcohol analyses. These same tubes <br />are used for blood draws and for drug analyses. Two-ounce urine collection containers <br />without preservative and tamper proof transfer bags are supplied for urine drug <br />screens. Both blood vacutainer tubes as well as urine containers are provided with <br />tape-seals that the officer can complete at the time of collection indicating the <br />subject's name, the date and time of collection, the officer's name, and the name of either <br />the urine collection witness or the person drawing the blood sample. These labels are <br />designed to go over the cap or stopper from the sides of the tube or container, as directed <br />by the Uniform Standards for Withdrawal, Handling and Preservation of Blood Samples for <br />Ascertain Forensics at RTL. <br /> <br />b) Chain of custody forms, security seals and boxes or envelopes for specimen shipment <br />are provided by Ascertain Forensics at RTL. The chain of custody forms include space <br />for the suspect's name, arresting agency,arresting officer,violation,date and timeof <br />sample collection, and other demographic information. Specimens for each case will be <br />placed in a cardboard box or evidence envelope and sealed with the provided security <br />seal. The box/envelope and corresponding chain of custody form will be placed into a <br />double pouch specimen baggie and sealed. Then placed in a large recloseable bag, that <br />bag will be placed into a bubble sealable pouch and then placed in an opaque, FedEx <br />lab pack and sealed. The FedEx shipping label will be placed onto the lab pack, and a <br />tracking number sticker from the label should be retained by staff. The FedEx bag can <br />be delivered to a FedEx hub or a drop off location – to minimize additional ‘touches’ of <br />the specimen/lab pack. The lab packs are then scanned by FedEx and shipped overnight <br />to Ascertain Forensics at RTL.
