<br /> <br />EXHIBIT A <br /> <br />Program and scope of work: <br /> <br />1. Track all existing and newly found or installed backflow assemblies <br />2. Track all testers, tester licensing, certifications, test kits and annual test kit calibrations, as <br />well as track the testing companies, employees, licensing, and certifications. <br />3. Send notifications of “test due” and “test past due” to the customer. 1st year educational tri- <br />fold brochure can be included at no cost. An optional “test due - final notice” can also be sent <br />via certified mail to the customer (only if approved by the utility, as there are additional costs associated <br />with this service…can be funded by increased test entry fee) <br />4. Send “test failure” notices for backflow tests that have been submitted to us indicating a test <br />failure. <br />5. We also include “Installation Due” & “Installation Past Due” notices. <br />6. Returned mail to us? Again, unlike others, we perform extensive research via our online <br />subscription services, update the site information, and resend notices to the new contact at no <br />additional cost. <br />7. There are never any licensing costs, software upgrade fees, new release expenses, hardware <br />costs…we are all-inclusive. <br />8. Your utility and testers do not need any special hardware, apps, or software to access your <br />online data. A basic computer workstation, laptop, tablet, smart-phone, etc. is all that’s <br />needed! <br />9. The utility’s responsibilities include only: <br />a. Final enforcement, should the water customer elect to ignore our repeated requests <br />b. Provide us with updated water customer information at a frequency that is convenient <br />for the utility. This process can be automated by software. <br /> <br />10. All letters, brochures, notices are fully customized to each water purveyor. <br /> <br />11. Once testers are registered, they are issued a user name and password. Data is entered into our <br />TrackMyBackflow.com website by using the Hazard ID assigned to each device or by serial <br />number and/or address. Each CCC hazard can be given a Hazard ID tag.
