<br />Quotes for Tree Removal & Stump Grinding Services <br />The City of Pleasanton’s Public Works Department, Parks Division is seeking a qualified <br />contractor to perform tree work services at the Ken Mercer Sports Park and Tennis Community <br />Park. The work consists of tree removal of existing 34 total trees and coinciding 34 total stump <br />grinding. <br />GENERAL PROVISIONS: <br />1. CONTRACT: Awarded Contractor for the lowest bid will have to submit a Maintenance <br />and Trade Services Agreement. Compensation will be based on the submitted “Bid <br />Form”. Contractor must have the following items in place for the execution of the <br />Agreement (see below). Once Agreement is executed staff will notify Contractor and <br />agree on a mutual start date. <br />Insurance: Contractor and must provide and maintain in full force for the duration of the <br />Agreement, a Certificate of Insurance naming the City of Pleasanton as additional <br />Insurer, with an Endorsement Letter for the minimum coverage of: Commercial General <br />Liability and Bodily Injury ($2 m), Automobile Liability ($2m), and Workers <br />Compensation and Employees Liability ($1m). <br />D.I.R.: Contractor will be subject to registration and provide current number with the <br />Department of Industrial Relations (DIR) per Senate Bill 854. The mandate applies to all <br />contractors and sub-contractors who intend to bid or perform work on public works <br />projects (as defined in the Labor Code). <br />a. For projects awarded on or after April 1, 2015, Contractor shall furnish the records <br />specified in California Labor Code section 1776, including but not limited to the <br />certified payrolls, directly to the Labor Commissioner. The Contractor shall furnish <br />the records specified in California Labor Code section 1776 to the Labor <br />Commissioner for all projects, whether new or ongoing, awarded on or after January <br />1, 2016.” Copies of those certified payroll records shall also be submitted <br />electronically to the City. <br />b. In accordance with California Labor Code Section 1170 et seq., the Contractor shall <br />pay general prevailing rate of per diem wages to all workers employed under this <br />contract. <br /> <br />City Business License: The contractor shall provide to the City of Pleasanton and a copy <br />of the vendor’s current City of Pleasanton business license. <br /> <br />W9: If the contractor is a new vendor, they must submit a recent W9. <br /> <br />2.
