Describe how the project/program is addressing the "Strategic Goals" and "Priority Needs" identified in the <br />"City of Pleasanton U.S. Department of Housing and Urban Development (HUD) FY 2020-2024 Consolidated Plan". <br />Report 1 <br />We address all 4 goals <br />Report 2 <br />We provided supportive housing with 24 hour mental health and harm reduction model trained staff. We have 28 single unit <br />private, dignified homes with 8 being ADA compliant and we are working with an architect for additional homes to be built <br />at the village. Individuals who reside at this intensely supportive village can remain at the village permanently however they <br />are still considered homeless in order to maintain eligibility to transition to a less supportive independent housing when <br />and if they feel able to maintain such housing. Each individual has a case manager and an individualized care plan to <br />identify areas of need to be strengthened in order to maintain housing in a less supportive environment. <br />15. Describe the original purpose for this grant. Will you spend your entire grant? If not, explain why your agency <br />did not spend the entire grant. <br />Report 1 <br />The grant provided $40,000 for 24 hour services. This is less than one staff member for our 24 hour program. We used the <br />funds. <br />Report 2 <br />This grant provided funds for our exceptional support staff. Stable staffing results in a stable village where the neighbors <br />are able to develop trust and begin to recover. Long term staff are also able to witness patterns and predict de- <br />compensation which enables them to intervene before the neighbor is in crisis. Instability leaves neighbors guarded, <br />withdrawn and delay recovery <br />
