Describe the accomplishments of the project or program in this reporting period, including details on how the <br />program responded to the needs within the community and any new and creative methods the agency <br />implemented to meet those community needs. <br />Report 1 <br />7 Pleasanton formerly unsheltered individuals retained supportive housing and are receiving wrap around services for <br />mental health, vocational skill building, and daily living support. <br />Report 2 <br />As of today we have 8 Pleasanton neighbors. 2 moved in to the village during this last reporting period. <br />Our village continues to maintain an individualized program determined by each neighbors individualized needs. We are <br />continually evaluating, strategizing and thinking creatively to meet the needs of this population. We continue to be low <br />barrier and highly supportive. <br />13. Describe how the project/program is addressing the critical human services needs in the "City of Pleasanton <br />Human Services Strategic Plan Needs Assessment Update 2018-2022". <br />Report 1 <br />Housing, connection to services, aging in place. <br />Report 2 <br />We provide supportive housing for those who meet the criteria of chronically homeless. We have 24 hour mental health <br />staff, we are low barrier for admission to the village and for housing retention. We are a harm reduction program and you <br />will not lose your housing due to use unless behaviors result in a safety concern for the village. We focus on restoring <br />connection to self and others, building self esteem, identifying strengths and developing healthy coping mechanisms. We <br />provide onsite crisis de-escalation to decrease use of emergency services.
