Pay Item 4 — Ultrasonic Pole Testing <br /> As requested by City, Contractor shall perform ultrasonic testing and report for each <br /> light pole requested. The ultrasonic testing shall be capable of locating the length, <br /> lateral location, and depth of defects. The inspector shall have a ASNT Level II <br /> certification (ASNT SNT-TC 1A). Certification shall be submitted as part of the RFP and <br /> shall also include ultrasonic equipment cut sheets. Report of inspection shall be <br /> submitted to City no later than 72 hours after completion of inspection. <br /> Ultrasonic Pole Testing shall be paid for each pole location, shall be inclusive of all work <br /> under section 4, and shall be paid for at the unit price of each. <br /> ADDITIONAL SERVICES - TIME & MATERIALS: <br /> EMERGENCY WORK/RESPONSE <br /> As requested by City, Contractor will be called to respond to accident sites, disconnect <br /> power, secure the site for public safety, take down damaged poles or pick up a knock <br /> downed light pole. Contractor shall furnish a price for performing this work both during <br /> normal working hours and during weekends, holidays, or other off-hours. The response <br /> time for emergency calls during business hours shall be no more than one (1) hour. The <br /> response time for emergency calls during standard, non-working hours, weekends, and <br /> holidays shall be no more than two (2) hours. An estimated arrival time by Contractor <br /> shall be provided upon request from City staff. Poles shall be moved to the OSC storage <br /> yard or placed in recycled bin if available at 3333 Busch Road, per direction of the City. <br /> Cost of the disposal, if directed by the City, shall be on Time and Material basis. <br /> All costs for labor and equipment to install a replacement pole and restore the light to <br /> proper operation shall be on time and material basis or proposal per the direction of City. <br /> Contractor shall furnish a price for reinstalling knockdowns with like materials within 24 <br /> hours. <br /> Contractor shall cooperate with the Police Department and the Operations Services <br /> Superintendent in cases of emergency. <br /> NIGHT CHECKS <br /> Contractor shall perform night checks upon request from City Staff. Night Checks shall <br /> include all work necessary for inspection of lights as directed by the City. Contractor <br /> shall submit the date of inspection/s and list of lights that are not working with pole <br /> number, type of fixture, and nearest address and anticipated date of repair. Contractor <br /> shall provide name and contact number of the lead staff performing the inspections. The <br /> report shall be received no later than 24 hours after the inspection. All costs shall be <br /> inclusive of all equipment and personnel necessary to conduct night checks and <br /> report. <br /> CONSULTING SERVICES <br />
