required the Caltrans permit was removed from this project by change order. The work <br /> is incorporated into the upcoming Annual Resurfacing of Various Streets Project, CIP <br /> No. 21503. The other change orders addressed minor alterations including additional <br /> striping, adjustments for crack sealing quantities and increases in the cost of oil for the <br /> asphalt in accordance with the contract documents. <br /> Econoloite, the City's on-call traffic contractor, installed video detection equipment at <br /> eight major signalized intersections within the limit of project for $35,422. Video <br /> detection equipment is installed to maintain operation of signals when vehicle detector <br /> loops in the pavement are disturbed during construction. <br /> Consulting Services <br /> BSK Associates, Inc., the City's on-call geotechnical consultant, provided material <br /> testing and verification of the density of the pavement in accordance with the City's <br /> quality assurance plan for $17,968. <br /> Alexander & Associates, Inc., the City's on-call surveyor, provided monument ties, <br /> documenting the location of monuments before and after construction to ensure they <br /> were not disturbed, for $1,025. <br /> The work has been completed to the Director of Engineering's satisfaction and it is <br /> recommended that the project be accepted as complete. <br /> Submitted by: Fiscal Review: Approved by: <br /> <4.44 <br /> /Ak AJV <br /> Stephen Kirk rick Tina Olson Nelson Fialho <br /> Director of Engineering Director of Finance City Manager <br /> Attachment: <br /> 1.
