Attachment 2 <br /> Exhibit A <br /> SCOPE OF SERVICES <br /> I. SERVICE OVERVIEW <br /> The mission of the City of Pleasanton's paratransit program (Pleasanton Paratransit Service, or <br /> PPS) is to provide safe and reliable transportation for seniors and individuals with disabilities <br /> so as to improve their access to community services and activities,to decrease their experience <br /> of social isolation, and to enhance their abilities to remain living independently in their chosen <br /> communities. <br /> PPS is designed to supplement and complement the ADA-mandated services provided by the <br /> Livermore Amador Valley Transit Authority(LAVTA). PPS provides the following services to <br /> eligible ambulatory and non-ambulatory program participants: <br /> • Door-to-Door Transportation <br /> • Shuttle Service (to be installed at a time to be mutually determined by both parties) <br /> Program parameters outlined in the scope of services apply to transportation services beginning <br /> February 1,2021. <br /> A. Door-to-Door Transportation <br /> PPS provides wheelchair accessible,door-to-door transportation to and from destinations within <br /> the City of Pleasanton, and to limited locations in the adjoining area, such as specific medical <br /> facilities in Dublin,Livermore and San Ramon for: <br /> a) City residents who are 70 years old and older,and <br /> b) City residents who are ADA paratransit eligible. <br /> Riders can purchase one way rides via vouchers (punch cards)through the CONTRACTOR or <br /> by any other means deemed suitable by the CONTRACTOR.The CONTRACTOR will manage <br /> the purchase of one way rides and all rider payment options will go through the <br /> CONTRACTOR. <br /> Riders can schedule trips from one to three days in advance. Riders may not request same day <br /> trip service. Rides are reserved on a first come, first served basis; trip requests are <br /> accommodated based on vehicle availability.Rider requests for standing orders must be directed <br /> to the CONTRACTOR for approval and scheduling. The CITY may instruct the <br /> CONTRACTOR to prioritize reservations at any point it deems necessary. For example, the <br /> CITY may allow medical trips to be prioritized over non-medical trips. <br /> One personal care attendant, who provides needed assistance to paratransit riders, may travel <br /> with program participants at no additional charge.
