Box 520 <br /> Pleasanton, CA 94566 <br /> Re: Community Development Block Grant (CDBG)—COVID-19 Emergency Funding <br /> Dear Becky: <br /> Thank you for your recent email outlining the new CDBG-COVID-19 emergency funding that may be <br /> available to Axis Community Health (Axis) in the amount of$130,314. As you requested, I have outlined <br /> a 24-month plan that describes how Axis would expend these funds if awarded to us by City Council. <br /> Under the Housing and Urban Development guidelines for the funding, Axis would be providing services <br /> related to activities that prevent and respond to the spread of infectious diseases such as COVID-19. <br /> These services would include: (1) the provision of both clinic-based and telehealth medical services to <br /> assess patients for symptoms of COVID-19 or other infectious diseases, (2) diagnostic, treatment and <br /> follow-up care for patients who are at-risk for or diagnosed with COVID-19 or other infectious diseases, <br /> (3) coordinating related ancillary services, to include testing, contact tracing, pharmaceuticals and home <br /> diagnostic equipment as appropriate, and (4) patient, family and community education regarding <br /> infectious disease prevention. In order to conduct these services, Axis will also procure necessary <br /> supplies, to include testing items and personal protection equipment. When antibody testing and <br /> treatment modalities as applicable to COVID-19 and other infectious diseases become available, Axis will <br /> develop and implement strategies to provide these enhanced services for patients as well. <br /> Axis will provide these services at its two Pleasanton clinic sites, located at 4361 Railroad Avenue and <br /> 5925 W. Las Positas Blvd. This project will serve approximately 2,400 Pleasanton residents who are <br /> patients at Axis; an estimated 1,775 (74%) of these patients will have a family income that is less than <br /> 100%of the Federal Poverty Level (FPL) and 480 (20%) will have a family income that is less than 200% <br /> of FPL. <br /> We appreciate the opportunity to apply for this support and would be happy to respond to any <br /> questions regarding this proposal.
