3) coordinating related ancillary services, to include testing, contact <br /> tracing, pharmaceuticals and home diagnostic equipment as appropriate, <br /> and <br /> 4) patient, family and community education regarding infectious disease <br /> prevention. <br /> To provide these services, Axis will procure necessary supplies, to include testing items <br /> and personal protection equipment. When antibody testing and treatment modalities as <br /> applicable to COVID-19 and other infectious diseases become available, Axis will <br /> develop and implement strategies to provide these enhanced services for patients as <br /> well. These services will be provided at the two Pleasanton clinic sites and will serve <br /> approximately 2,400 Pleasanton residents. An estimated 74% of these patients will <br /> have a family income that is less than 100% of the Federal Poverty Level (FPL) and <br /> 20% will have a family income that is less than 200% of the FPL. <br /> Spectrum Community Services will spend $48,000 over a 15-month period by delivering <br /> meals to COVID-19 related clients who are seniors that would not otherwise qualify for <br /> the program and disabled residents under the age of 60 who are unable to prepare or <br /> access food. This is an expansion of their current program and it is estimated that 15 <br /> individuals in Pleasanton will be served approximately 4,500 meals over the 15-month <br /> period. At $10 per meal, the cost is $45,000. <br /> The remaining $25,000 will go toward the Tri-Valley Regional Testing Center. The City <br /> Council committed $100,000 from the general fund for the Regional Testing Center of <br /> which the City intends to seek FEMA reimbursement for. FEMA will not pay more than <br /> 75% of the cost, leaving $25,000 remaining of which will be paid by the CDBG-CV <br /> funds. <br /> Attachment 3 includes the DRAFT Substantial Amendments to the FY 2015-2019 <br /> Consolidated Plan and FY 2019-2020 Annual Action Plan, which identifies the COVID- <br /> 19 activities to be funded with CDBG-CV funds, and the amendment to the Citizen <br /> Participation Plan. <br /> Public Review <br /> The DRAFT Substantial Amendments have been available for review for a 5-day public <br /> comment period (May 11, 2020 through May 15, 2020) as required by HUD under a <br /> current waiver regarding public comment and hearings, which were previously <br /> discussed. Staff recommends that the Council review the Substantial Amendments, <br /> provide comments as deemed appropriate, hold a public hearing to solicit any public <br /> testimony on the Substantial Amendments, and approve the documents for submission <br /> to HUD for review and approval. HUD has advised that the City may access its CDBG- <br /> DV funds as soon as its Substantial Amendments are approved. <br /> Page 3 of 4 <br />
