Human Services Needs Assessment Strategic Plan 2014-2016, the Human Services <br /> Commission recommended the strategic action: Revise Human Services Commission <br /> Grant-making Protocols/RFPs, including exploration of multi-year contracts with core <br /> safety-net service providers. On February 5, 2019 the City Council adopted the City of <br /> Pleasanton Human Services Needs Assessment Updated Strategic Plan 2018-2022, <br /> reaffirming the strategic action. <br /> At the August 7, 2019 meeting, the Human Services Commission identified Health Care <br /> and Behavioral Health as the most critical human service need to address through a <br /> multi-year agreement and a RFQ process. The Commission also recommended that <br /> $35,000 be set aside annually from the Housing and Human Services Grant (HHSG) <br /> process to fund this service. <br /> On December 4, 2019, the Human Services Commission approved a motion to focus <br /> the Request for Qualifications (RFQ) on Behavioral Health Services. Staff then <br /> advertised the RFQ, screened applicants, conducted interviews and provided a <br /> recommendation to the Commission to award a multi-year Agreement for Services to <br /> Axis Community Health to provide behavioral health services in the community. The <br /> annual contract award is for $35,000, renewable for up to five years. The total <br /> Agreement for Services amount is not to exceed $175,000. (Attachment 7) <br /> Housing Grants <br /> In past years, the City has utilized its LIHF funds to supplement limited CDBG program <br /> funding for important housing-related services. Federal HOME Funds received through <br /> the Alameda County HOME Consortium have also historically been appropriated through <br /> the HHSG Process. Housing services applications were reviewed by the Housing <br /> Commission at its April 30, 2020 meeting and the Commission's recommendations are <br /> summarized in Table 4 resulting in the recommended allocation of$188,305 in HOME <br /> funds and $264,196 in LIHF funds for FY 2020/21. <br /> Page 8 of 12 <br />
